substance, drug (other than with a pre-approved valid prescription) or related
Possession or consumption of any alcoholic

Violation of training rules, as stipulated
by the coach or Program Director. This includes, but is not limited to, violations
such as lateness for bus departure times, practice and/or games.
Arrest or detention for violation of
local laws or ordinances.
Any action which may threaten the health
and safety of oneself or another.

may be issued certain pieces of equipment that will be his responsibility to maintain
during the program. All equipment not designated as part of the player “uniform
package” must be returned to the Program Director at the end of the program. This
list includes, but is not limited to, items such as team helmets, catcher's gear,
baseballs, bats, etc.
I have read
and approved the Rules of Conduct described above. I understand and affirm that
this list is not meant to be all-inclusive and that general positive behavior
is expected at all times. I understand that any violation of these Rules of Conduct
will result in my immediate removal from the program and return to the United
States at my own expense.
As the parent/guardian
of the aforementioned player, I have read and approved the Rules of Conduct described
above. Furthermore, I understand that my son will be immediately expelled from
the program and returned home at his own expense if he is found in violation of
any of these rules.
